Guidelines For JFS Caregiver Assistance Program Agreement Between Care Receiver And Jewish Family Service

Introduction
The following represents guidelines for the administration of the JFS Caregiver Assistance Program. The Program is being funded through the auspices of the Foundation for the Jewish Elderly of Eastern Pennsylvania. The goal of the Program is to fill a niche in service capacity in offering the opportunity for needy Jewish senior citizens, 55 years and older, who cannot afford to pay for temporary unskilled care in Wyoming Valley and who live in the Wyoming Valley.

Qualifications
The individuals who apply for such assistance are those who do not qualify under the various programs of the Area Agency on Aging for Luzerne and Wyoming Counties or any other support program that otherwise could provide the type of assistance being offered under the JFS Caregiver Assistance Program. The Care Receiver can only receive the funding under this Program in his or her primary residence.

Definition
Temporary unskilled care is defined as the employment of an individual to attend to the needs of the individual in his or her home or apartment that relate to personal care such as cleaning, bathing, shopping, and other needs that do not require the type of support that otherwise would be defined as skilled care.

Equipment
Where equipment or an assistive device is requested that can be provided under this Agreement, such equipment is limited to no more than $300 per person being served and must be agreed to between JFS and the care recipient. The equipment may be purchased or rented, depending upon the needs of the individual and JFS will make the check payable to the vendor for purchase or rental. The care recipient must establish that he or she cannot obtain the equipment through any other source, purchase or donated. Whenever it is appropriate and the equipment is no longer needed by the care recipient, said equipment is to be donated to JFS so that it can be used by other clients, so long as the equipment is of the type that can be utilized by more than one person.
